- Care Management Programs:
AvMed's Care Management Programs address the needs of members with
chronic conditions, specialty care needs or complex, life threatening
illnesses. A team of registered nurses is available to
help these members navigate the healthcare system and maximize their
benefits. Each eligible member is assigned a registered nurse, or
case manager. Through education and individualized attention,
AvMed empowers its members to take an active role in managing their
health. Our case managers work with physicians to coordinate a
member's care and help them make positive lifestyle choices and educated
healthcare decisions.
- AvMed’s On Call: A toll-free, confidential telephone health information
line available 24-hours a day, seven days a week and an audio health
information library with more than 400 topics.
- Weight Watchers Program:
Reimbursement for all fees paid to Weight
Watchers for a period up to one year by AvMed members who reach and maintain their goal weight as
defined in the Weight Watchers program. Click on the 'Weight Watchers' button to the left for
full details.
- Smokenders: An easy-to-follow method for kicking the smoking habit which
includes a discount rate for Smokenders and reimbursement if you quit smoking.
