Disease Management

This program assists members who have chronic medical problems. Our Disease Management Program empowers members living with heart failure, asthma or high-risk pregnancies to take back control of their health. We communicate with these members through telephone calls and home visits by registered nurses, by communicating with their physicians, through libraries of print and video information, and by medical equipment delivered to their homes. By teaching individuals with chronic health problems how to control their illnesses, we’re giving them new dimensions of power.

With a custom-made plan to fit their lifestyles and personal instruction on controlling symptoms, more people are living comfortably while managing their long-term medical conditions. AvMed nurses coordinate the care delivered to members by home care agencies, primary care physicians, and specialists.

Outcomes in our Disease Management Program range from the birth of a healthy baby to reducing the risk of hospitalization for a member with congestive heart failure. New programs will be implemented soon in diabetes, coronary artery disease, and respiratory problems.
