If you are already an AvMed Member, but have forgotten your password or need to create an account, please visit our Member Registration Page. In order to proceed, we require a valid email address and either your member/group number or your social security number. If you are a provider, please call AvMed's Provider Service Center at: (800)452-8633, or send an e-mail to: providers@avmed.org for your Username and Password. As an AvMed agent or a prospective agent, you do not need a login and password to gain access to our Agent & Broker portal. Compensation Statements are available through a login screen. To register for online services please contact: Large Groups contact your Client Service Representative. Small Groups Send written request to Premium.Services@Avmed.org and include the name of the Contact Person, E-mail Address and Phone number.
