We are pleased to announce AvMed’s 2012 High Performance Network (HPN). The information below
provides you with vital updates regarding selected specialties for HPN profiling, and our quality
and cost efficiency criteria and methodology. We encourage you to become familiar with this
information, as it affects you as our network partner and the AvMed Medicare members you service.
Member Cost Share Responsibilities:
Effective January 1, 2012, Medicare Advantage members who visit HPN
designated physicians** will incur a $0 co-payment, while members
visiting all other in-network physicians will incur a $25 co-payment.
