Appointment Forms

Once you have sold your first AvMed account, all you have to do to get appointed with us is complete a few forms and send them to your sales rep. or to Agent Services (9400 South Dadeland Blvd., Suite 315; Miami, FL 33156). AvMed can appoint you with the Department of Insurance at no cost to you.

Here’s a list of the forms you’ll need to complete and send to us before getting appointed:

• Application for Agent Appointment
• Direct Deposit Authorization Form
• IRS W-9
• Individual Producer Agreement
• Commercial Group Agent Agreement
• Copy of Errors & Omissions (E&O)
• Copy of Florida insurance license

For Individual Under 65, you must first get appointed before you can quote and write business. Please sign the Individual Under 65 Producer Agreement and appointment documents and fax to: 305-671-0135 or scan and email to agentsupport@avmed.org
